It was about two years ago since I first felt it. My right index finger started hurting after a session of excessive clicking. I learned to use the mouse in my left hand, but now I just use my right hand again. It might be the way my desk is positioned, or, even though I'm now comfortable navigating a gui with my left hand, it's still horrible in fps games (which I play every few weeks.) It's annoying to keep moving the mouse right and left (I only have one optical mouse. If I put another ball mouse, which is non-usb, my computer will wake up every time I move the mouse, which drives me nuts.
